Lorelei Sawchuk is the Education Lead and Nurse Practitioner with Covenant Health Palliative Institute. Lorelei is an active clinician working as the Palliative Care Nurse Practitioner and Co-Clinical Lead with the Covenant Health Palliative Care Consult Team for the Grey Nuns and Misericordia Community Hospitals and Villa Caritas. She also worked as a NP at the Edmonton General Continuing Care Centre for three months during COVID-19. Lorelei assisted in creating the Care in the Imminently Dying Pathway and was the Topic Lead for the Alberta Provincial Clinical Knowledge Topic Care of the Imminently Dying (Last Hours to Days of Life) Adult in All Locations.
This interactive 60 minute presentation will provide a review of the Care of the Imminently Dying Pathway and its resources and pilot evaluation findings one year post implementing the Pathway in Covenant Health's 17 rural and urban acute and continuing care sites in Alberta. Additionally, opportunities on how to successfully integrate implementation and sustainability of the Pathway will be discussed as the Pathway is now available through Alberta Health Services and Connect Care.
