Abstract/Description

As patients approach the end of life, taking medications for prevention or management of chronic disease may no longer be appropriate and contributes to polypharmacy. This is a time to review therapies and start eliminating unnecessary agents. Reviewing current literature we will describe rationale for and barriers to deprescribing. Patient cases will demonstrate how this may be achieved in a patient-centered fashion while maintaining effective symptom control and rational pharmacological strategies, thus “curing” polypharmacy.

 

Objectives

Describe guidelines or strategies to identify potentially inappropriate medications (PIMs).

Discuss the rationale for and barriers to deprescribing identified agents.

Demonstrate techniques to reduce polypharmacy in a patient-centered fashion.
